ConnectionsPlus has been designated as a Hospice CAHPS Honors Elite recipient by MatrixCare, a recognition awarded to topperforming hospice agencies nationwide.
This honor places ConnectionsPlus among an elite group of hospices that have demonstrated an unwavering commitment to delivering exceptional patient and caregiver experiences, said a spokeswoman.
The Hospice CAHPS Honors Elite distinction is based on results from the Hospice Consumer Assessment of Healthcare Providers and Systems (CAHPS) survey, which evaluates patient and caregiver satisfaction across a range of quality indicators. To achieve elite status, a hospice must outperform the Matrix-Care National Performance Score on all 24 evaluated quality measures.
ConnectionsPlus is one of only two hospices in the state of Virginia to receive this elite designation for the 2024 evaluation period, and the only five-star hospice in this service area by the Center for Medicare and Medicaid Services.
